From time to time it might be necessary to refer a patient to a consultant for further care. In such cases we will do our utmost to ensure that referral letters are sent promptly from the practice and wherever possible, via Choose & Book.
Your responsibilities to us are:
- to ensure that you keep the practice and the hospital informed of any changes of address and/or name so that hospital mail will reach you at the correct address
- to recognise that any delays in receiving an appointment are usually outside our control
- to keep your hospital appointment or cancel it in good time so it can be given to somebody else on the waiting list
